What is the success rate of FUE Hair Transplant procedure?The regrowth rate of transplanted grafts by FUE is more than 90%. However, we must bear in mind that all hair loss occurs over time. This includes hair loss in the donor area. Accordingly, since transplanted follicles are from the donor area, regardless of the positive outcome of hair restoration surgery, one may experience a 10-15% loss of the transplanted hair as well. However, this is over a lifetime. Therefore, the idea of a “safe donor” area is unfounded. Hair transplantation is an art, the art of illusion. As in all aesthetic surgery, there is no standard, nor is there a guaranteed satisfactory result for everyone for the whole lifespan. We are essentially all just trying to steal some years away from real life.

When is the appropriate time to have hair transplantation surgery?Some patients present themselves for surgery with very early hair loss and a significant amount of miniaturization. In a situation such as this, the better solution is to treat the condition with medication and to wait until a transplant is necessary. However, with recent improvements, there is no need to wait until hair loss has progressed to such an advanced degree that it causes emotional distress and anxiety.

How long does hair transplant last? It is of great importance to remember that there is no standard, nor is there a guaranteed satisfactory result for everyone in all aesthetic surgeries. This is true over the entire lifespan. Hair transplantation is mostly about the art of illusion. The average density of the frontal area in someone with no hair loss is a minimum of 70 follicular units per cm2, however, a high density looking hair transplant is often a result achieved with 50 follicular units per cm2. With 50 cm2, it is certainly possible to achieve the appearance of normal density when a proper surgical plan and graft ion is implemented. Furthermore, it is of the utmost importance to bare in mind that the idea of a “safe donor” area is deceptive. One can experience a 10-15% loss of the donor hair over a lifetime as well. Likewise, this loss may affect the transplanted hair and should be considered a natural phenomenon. In my professional opinion, a successful hair transplantation lasts for at least 10 years.

What is the best method of harvesting hair, FUE or the motorized technique?Let me begin my response by saying that there is no single technique or device that stands out as being the absolute best when in comes to hair restoration, just as there is no single surgeon that is the best in the world. Each surgeon develops his/her skill using his/her instruments of choice for the best possible surgical outcome. Not that long ago, I collaborated with Hacettepe Medical University in Turkey in a study using an electron microscope to determine the damage caused to graft quality using high rpm’s (30,000) with a motorized system. The results of the study showed that there was no real difference in graft quality between grafts extracted through a motorized system and those of a manual punch. Though the findings of the study showed no real difference in graft quality between the two techniques, I believe that the manual punch creates less damage to the donor area, thereby increasing the donor capacity in a positive way. In my own personal practice, I have chosen to use the manual punch combined with a technique that I developed, the DES or Sequential Method. Two of the state-of-the-art concepts in FUE are: 1) how to utilize the donor area in an efficient and effective way, and 2) how to create the best overall homogenization that allows for a natural look. Essentially, as FUE hair restoration specialists, we have solved the problems surrounding regrowth, density and a natural appearance. However, we are still facing some problems when it comes to donor capacity, especially in patients characterized as Norwood 5-6.

What is the maximum number of grafts that can be extracted in one session ?The maximum number of grafts that may be extracted from the donor in one session depends on several factors: donor capacity, calculated density, density, caliber and number of hairs per follicle. These factors are calculated from all parts of the donor area (parietal, temporal and occipital) and are combined in what I call Coverage Value. Other factors must also be taken into consideration such as DUPA, miniaturization and future hair loss in the donor area. (Most people experience up to 25% progressive hair loss over their lifetime, therefore the idea that there is a “safe donor area” is a misconception). All these factors must be considered in order to maximize density and homogenization. It is my professional opinion that the average donor capacity has risen to from 6,000-10,000 grafts. This is due to improved techniques and instrumentation as well as expertise on the part of the hair surgeon who is well trained and experienced in FUE. I began practicing the FUE technique using the manual punch (which I still use exclusively) in 2004 and have since that time, performed surgery in over 1000 cases with over 5000 grafts in a single session. (A session constitutes 2 days of surgery with this graft number). The maximum number of grafts that I have been able to extract in a single session is 6,500, however, I have performed surgery on 12 patients with a 10,000-graft extraction over several sessions. Earlier on, from 2000-2007 I performed the FUT technique as well as the FUE technique and at that time, I accepted the common opinion that the regrowth rate using FUT was greater than that of FUE. Since 2007, I have determined that this to be untrue. I have witnessed amazing growth rates in FUE in my own practice that equal and surpass those of FUT. The key is the experience of the FUE surgeon.

What is shedding (shock loss) after surgery?Some shedding (effluvium) after hair transplantation is a relatively common occurrence. It is noticeable beginning from the week following the surgery, and can continue for up to six weeks. Some patients do not shed, and the transplanted hair continues to grow. In general, those patients who have experienced more miniaturization and more rapid hair loss will most likely experience shedding from the surgery. For the majority of patients, it is a minor issue and should not be the cause for concern. Finasteride use may decrease the risk of shedding following hair transplants.

Smoking and hair transplant?It is believed that smoking affects the proper amount of blood flow to the transplanted grafts. Therefore, patients are recommended to avoid smoking or to limit smoking prior to the surgery, and particularly one week after the procedure. During the period immediately following surgery, graft re-vascularization occurs creating an ideal environment for the grafts to survive and grow as well as receive nutrients by diffusion. Consequently, avoidance of smoking during this period is of great importance to ensure an adequate supply of oxygen and nutrients to the transplanted grafts.

Is it possible to create a new hairline even if you have no hair loss? It is certainly possible to create a completely a new hairline. When reconstructing a new hairline, it is of great importance to take into consideration factors like donor capacity, facial traits and hair type. I usually place original fine single hair grafts in the frontal hairline in order to achieve a more natural and satisfactory result.
