How can you maximize hair density of a strip scar by FUE? Sometimes density alone over the scar area may not be enough to create the ideal cosmetic aspect desired. This is due to the color contrast between the skin and the scar tissue. A better solution would be a combination of FUE and micro-pigmentation. With exceptionally large scars and elastic skin found in some cases, and to avoid the wasting of valuable grafts, I would suggest scar revision with an experienced doctor prior to FUE and pigmentation.

Why manual FUE?We continue to closely monitor the development and advancement of new technology that can help to improve our results. We are convinced that the FUE process is the most effective technique that has been developed until now. With manual FUE, we are able to control the procedure during its execution. We are also able to maintain control of the adjacent area, so that we can preserve and use the donor area as effectively as possible. The examination of each graft for its quality prior to extracting another is possible with FUE and every moment of the procedure can be adjusted and changed as it is deemed necessary. Likewise, this loss may affect the transplanted hair and should be considered a natural phenomenon. In my professional opinion, a successful hair transplantation lasts for at least 10 years.

When is the appropriate time to have hair transplantation surgery?Some patients present themselves for surgery with very early hair loss and a significant amount of miniaturization. In a situation such as this, the better solution is to treat the condition with medication and to wait until a transplant is necessary. However, with recent improvements, there is no need to wait until hair loss has progressed to such an advanced degree that it causes emotional distress and anxiety.

Smoking and hair transplant?It is believed that smoking affects the proper amount of blood flow to the transplanted grafts. Therefore, patients are recommended to avoid smoking or to limit smoking prior to the surgery, and particularly one week after the procedure. During the period immediately following surgery, graft re-vascularization occurs creating an ideal environment for the grafts to survive and grow as well as receive nutrients by diffusion. What is the best method of harvesting hair, FUE or the motorized technique?Let me begin my response by saying that there is no single technique or device that stands out as being the absolute best when in comes to hair restoration, just as there is no single surgeon that is the best in the world. Each surgeon develops his/her skill using his/her instruments of choice for the best possible surgical outcome. Not that long ago, I collaborated with Hacettepe Medical University in Turkey in a study using an electron microscope to determine the damage caused to graft quality using high rpm’s (30,000) with a motorized system. The results of the study showed that there was no real difference in graft quality between grafts extracted through a motorized system and those of a manual punch. Though the findings of the study showed no real difference in graft quality between the two techniques, I believe that the manual punch creates less damage to the donor area, thereby increasing the donor capacity in a positive way. In my own personal practice, I have chosen to use the manual punch combined with a technique that I developed, the DES or Sequential Method. Two of the state-of-the-art concepts in FUE are: 1) how to utilize the donor area in an efficient and effective way, and 2) how to create the best overall homogenization that allows for a natural look. Essentially, as FUE hair restoration specialists, we have solved the problems surrounding regrowth, density and a natural appearance. However, we are still facing some problems when it comes to donor capacity, especially in patients characterized as Norwood 5-6.
