La calvizie maschile, detta alopecia androgenetica, porta alla caduta dei capelli prima presenti sul cuoio capelluto. Si verifica a seguito degli importanti cambi ormonali che avvengono nel corso della vita, in particolare negli anni successivi all’adolescenza. Sebbene la caduta dei capelli rappresenti una parte normale della fase di invecchiamento per milioni di persone in tutto il mondo, essa può significare comunque un evento psicologicamente difficile ed angosciante emotivamente, e può rendere necessaria la messa in atto di una serie di terapie e trattamenti. Disturbi nutrizionali o una cattiva alimentazione, problemi di salute, alcuni fattori neurologici e ormonali sono tra i motivi che portano alla caduta. La scala Norwood è il metodo di valutazione principale utilizzato per definire il grado di calvizie maschile attraverso degli schemi di caduta di capelli. Come misura si utilizza una scala da 1 a 7, che facilita l'identificazione di varie tipologie di calvizie attraverso dei modelli di riferimento. È considerata la misura più utilizzata dai medici quando si affronta la calvizie maschile. La scala di Norwood aiuta a capire il grado di caduta dei capelli, indirizzare il trattamento appropriato e la valutazione dei risultati.
Il grado 5 della scala Norwood è considerato come un livello avanzato della calvizie maschile. In questa fase, il modello di caduta può essere descritto come la presenza di pochi capelli o nessun capello rimanente, in particolare nella regione della chierica. Le aree dell'attaccatura e della chierica sono separate da capelli estremamente radi; il ponte che collegava l'attaccatura con la chierica si è ristretto e assottigliato e il classico aspetto “a ferro di cavallo” diventa più visibile.
Quando viene diagnosticato il quinto livello Norwood, il paziente si trova ad un punto critico. Va tenuto presente che se i medici hanno eseguito una valutazione adeguata, non significa che si evolverà necessariamente verso una calvizie completa. Generalmente, a questo punto, il trattamento più efficace è una procedura di trapianto mirata a ripristinare nuovamente la capigliatura.
In questa fase, la calvizie diventa più difficile da gestire in modo efficace mediante i trattamenti tradizionali come il Minoxidil. Raggiunto questo livello, è possibile che abbiano luogo alcuni anni di stabilizzazione. Tuttavia, se non viene seguita una terapia adeguata molta probabilmente si svilupperà una forma di calvizie più avanzata attraverso un'ulteriore evoluzione relativa alla scala Norwood.
In questa fase, non è possibile determinare il periodo esatto in cui una persona potrà mantenere una qualità stabile di capelli. Considerati vari fattori ambientali, fisici e fisiologici, potrebbe permanere durante mesi o anni a seconda della natura del problema. Dopodiché se non è stata intrapresa una terapia adeguata, si continuerà sicuramente verso una forma di calvizie avanzata.
Il Norwood 5 viene considerato come un livello di calvizie avanzato. Rispetto alle fasi iniziali esso necessita di un numero maggiore di innesti per coprire le aree calve. In generale, in questo livello, potrebbero essere necessari da 2.000 a 3.000 innesti per la parte anteriore e tra 1.000 e 1.500 innesti per la chierica. In alcuni casi avanzati di Norwood 5, potrebbero essere necessari 2500, 3200 o più innesti. È importante capire che il numero esatto di innesti necessari per il trapianto possa essere determinato solamente dopo avere consultato un medico specialista.
Senza dubbio, si tratta infatti della procedura più adatta per questa fase. Più velocemente si agisce, maggiori sono le probabilità di evitare che le zone diventino calve. Generalmente, in questa fase, i medici suggeriscono di sottoporsi a un trapianto, per via della sua efficacia nel ripristinare i capelli persi e prevenire la perdita degli stessi. La maggior parte degli interventi chirurgici di trapianto di capelli che vengono realizzati in questa fase ha risultati eccellenti se viene applicata una metodologia adeguata. Attualmente la procedura FUE è considerata la più efficiente nei casi Norwood 5. L'estrazione delle unità follicolari (FUE) è considerata la procedura di microinnesto più sofisticata poiché le unità follicolari vengono rimosse una per una con le loro strutture perifollicolari senza bisogno di tagli.
