La calvicie de patrón masculino, llamada alopecia androgenética, lleva a la pérdida de cabello en el cuero cabelludo. Es el resultado de los importantes cambios hormonales que ocurren a lo largo de la vida, particularmente en los años después de la adolescencia. La escala de Norwood hace facilmente comprender el grado de pérdida de cabello, indicar el tratamiento adecuado y evaluar los resultados.
El quinto grado en la escala de Norwood se considera un nivel avanzado de calvicie de patrón masculino. En esta etapa, el patrón de caída puede describirse como con poco o ningún cabello restante, particularmente en la región de la coronilla. Las zonas de la primera linea y la coronilla estan separados por un cabello extremadamente escaso; el puente que conectaba la zona frontal con la posterior se ha diminuido en superficie y densidad y la clásica esquema de herradura se hace más visible.
Cuando se diagnostica el quinto nivel de Norwood, el paciente se encuentra en un punto de inflexión. En esta etapa, parar la calvicie se hace más difícil con tratamientos tradicionales como Minoxidil. Una vez alcanzado este nivel, es posible que haya estabilidad en la caida durante algunos años. Sin embargo, si no se sigue la terapia adecuada, lo más probable es que se desarrolle una calvicie más avanzada a través de una mayor evolución en la escala de Norwood.
En esta etapa, no es posible determinar el período exacto en el que una persona podrá mantener estable la calidad del cabello. Dados varios factores ambientales, físicos y fisiológicos, podría persistir durante meses o años dependiendo de la naturaleza del problema. Después de eso, si no se ha realizado la terapia adecuada, ciertamente continuará hacia una forma avanzada de caida de pelo.
Norwood 5 se considera un nivel avanzado de calvicie. En comparación con las etapas iniciales, requiere un mayor número de injertos para cubrir las zonas calvas. En general, en este nivel, se pueden requerir de 2,000 a 3,000 injertos para la parte anterior y entre 1,000 y 1,500 injertos para la coronilla. Actualmente el procedimiento FUE se considera el más eficiente en los casos de Norwood 5. La extracción de unidades foliculares (FUE) se considera el procedimiento de microinjerto más sofisticado ya que las unidades foliculares se extraen una a una con sus estructuras perifoliculares sin necesidad de corte. Es mínimamente invasivo y las cicatrices que quedan, una vez curadas, son casi invisibles.
